You can have John's Publico or Henry meet you at the airport and take you to wherever you need to go. They charge by the trip, and I think last spring it was $70 plus tip. The contact information for John and Henry is on this site under "transport" "Getting There" "Getting to Fajardo". We've used John twice and he has been great both times. It may not be the cheapest way, but you know you will have someone waiting for you. They can also stop along the way for food, supplies, etc.
Renting a car is also an option. I have found the best deals with Enterprise, but others recommend Avis. You can pick up the car in San Juan and drop it off in Fajardo. The rental car company will take you to the ferry docks if you make arrangements ahead of time. However, with a 4:05 p.m. flight you'll want to be sure the rental car company's hours will accomodate your schedule if you decide to go this route.
We have not tried just getting a taxi to Fajardo, although we have used taxis to get around San Juan and have found them to be readily available. Perhaps others can speak to the wisdom of waiting until you arrive and hailing a cab to Fajardo.
